    A $20 booking fee is required at the time of scheduling. The remaining balance is due at the time services are rendered.

  • If the blood draw is unsuccessful, the remaining balance will not be charged. The $10 booking fee is non-refundable unless cancellation is made within the required timeframe (see cancellation policy below).

  • Your safety and comfort are a priority. If you experience dizziness, nausea, or discomfort, the procedure will be paused at once, and appropriate care will be provided.

  • Your healthcare provider must provide you with a laboratory requisition form. You must have the completed requisition form at least 24 hours prior to booking your appointment.

  • I am HIPAA compliant. All personal and health information is handled with strict confidentiality in accordance with federal privacy regulations.

  • Yes. Insurance information must be included on the laboratory requisition form, as required by the lab processing your specimen.

  • I have over 25 years of experience in phlebotomy.

  • Appointments must be canceled at least 24 hours in advance to receive a refund of the $20 booking fee. Cancellations made less than 24 hours before the scheduled appointment are non-refundable.

Policies and Term of Service

Description of Services

Healing Hanz ParaMedical Srvcs LLC provides mobile paramedical phlebotomy services, including but not limited to:

• Blood specimen collection

• Urine specimen collection

• Saliva or other approved specimen collection

• Vital signs measurement (if required)

• Specimen preparation and delivery to designated laboratories

• Chain-of-custody handling when required

• Home Health care

• EKG Imaging

Services are provided at the Patient’s residence, workplace, or agreed location. Healing Hanz ParaMedical Srvcs LLC performs specimen collection only and does not provide medical diagnosis, treatment, or medical advice.

By signing this Agreement, the Patient:

• Authorizes Healing Hanz ParaMedical Srvcs LLC to perform specimen collection as requested by the ordering physician, laboratory, insurance company, or authorized third party.

• Confirms that they have received instructions (if applicable) regarding fasting, medication restrictions, or preparation requirements.

• Acknowledges that failure to follow preparation instructions may result in specimen rejection and additional fees.

A $20 non-refundable booking fee is required to secure your scheduled appointment. This fee must be paid after submitting the booking form and when your appointment is confirmed.

The booking fee will be applied toward your total service cost. Deposits are non-refundable under all circumstances.

The Patient understands that:

• Phlebotomy and specimen collection involve minor risks including bruising, bleeding, fainting, infection, nerve irritation, or discomfort.

• Although rare, complications may occur.

• Healing Hanz ParaMedical Srvcs LLC personnel are trained professionals performing procedures within their scope of practice.

The Patient voluntarily consents to specimen collection services.

 The Patient agrees to:

• Provide accurate identification and medical information.

• Inform the phlebotomist of any relevant medical conditions (e.g., bleeding disorders, fainting history, and allergies).

• Provide a safe, clean, and accessible environment for the procedure.

• Ensure pets are secured during the appointment.

• Follow all post-procedure instructions.

Healing Hanz ParaMedical Srvcs LLC reserves the right to refuse service if conditions are unsafe or inappropriate.

• Blood Draw: $50-65

• Specimen Collection: $30

• EKG Imaging: $60

• Specialty Test: $75 - $100

• Travel (if applicable): $10

• Booking: $20

• After-hours (if applicable): $20

• Cancellation: $20

• Same-Day Service $25

• Additional Clients (if applicable) $40

 Payment terms:

• Payment is due at the time of service unless otherwise arranged.

• Healing Hanz ParaMedical Srvcs LLC does not guarantee insurance reimbursement.

• Returned payments are subject to additional fees.

Travel fees may apply depending on the distance to your service location. The total cost, including any travel fee, will be communicated prior to your appointment.

Appointments are not confirmed until the booking form has been completed and the $20 non-refundable booking fee has been received.

Appointments canceled less than 24 hours before the scheduled time may incur a cancellation fee.

Missed appointments without notice may incur a full-service charge/ non-refundable $20 booking fee

Healing Hanz ParaMedical Srvcs LLC complies with applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA), if applicable.

Patient health information will be:

• Used only for service delivery and required reporting.

• Shared only with authorized laboratories, physicians, or requesting entities.

• Protected according to industry standards.

Healing Hanz ParaMedical Srvcs LLC is not responsible for:

• Laboratory errors or delays once specimens are transferred.

• Delays due to incorrect patient information.

• Rejection of specimens due to patient non-compliance.

• Indirect, incidental, or consequential damages to the extent permitted by law.

Liability is limited to the amount paid for services rendered.

Healing Hanz ParaMedical Srvcs LLC does not provide emergency medical services.

If a medical emergency arises during the appointment:

• Emergency services (911) may be contacted.

• The Patient agrees to assume responsibility for emergency response costs.

Healing Hanz ParaMedical Srvcs LLC personnel are independent contractors or employees acting within the scope of permitted services and are not primary care providers.

This Agreement shall be governed by the laws of the State/Province of Pennsylvania  

The Patient agrees to release and hold harmless Healing Hanz ParaMedical Srvcs LLC, its employees, and contractors from claims arising from:

• Ordinary risks associated with specimen collection.

• Patient failure to disclose medical conditions.

• Unsafe conditions at the service location.

This does not waive liability for gross negligence or willful misconduct.

This document constitutes the entire agreement between the parties and supersedes prior communications.

• They have had the opportunity to ask questions.

• They voluntarily consent to services.
